Night-shift falls are chaotic for families. The useful move is the same every time. Get safe care first. Then pull the paper trail while it still exists.

This is a records checklist for Arizona families. It is not a diagnosis of any named home.
See your person, then freeze the first story
Go in person if you can. Ask to see the fall location. Ask who found your family member and at what time.
Photograph visible injuries in plain light before new bandages hide the skin. Take dated photos as bruising changes.
Write down the nurse’s first explanation word for word. Note whether the story changes on the next shift.
Ask whether anyone completed a facility incident report. Request a copy the same night or the next morning.
Ask what time the fall happened and what time a doctor or on-call provider was notified. Those two times matter.
The chart pages that carry the most weight
Request the full medical chart for the stay, not a one-page summary. Ask for nursing notes, medication administration records, and vital-sign logs around the fall.
Fall-risk assessments and care-plan pages show what the facility already knew. Families should ask for both.
Request therapy notes if physical or occupational therapy was involved. Request prior fall history inside the facility.
Ask for staffing assignment sheets for that shift if the facility will release them. Even a simple list of who was on the unit helps later questions.
Keep every hospital transfer record if your person went to an ER. The ambulance face sheet belongs in the same folder.
Public CMS files you can open without permission
Every Medicare-certified nursing home has a public profile on CMS Care Compare. Search by name, city, or zip at medicare.gov/care-compare.
CMS updates the star rating monthly on health inspections, staffing, and quality measures. Check the current rating, not last year’s brochure.
One star means much below average under the CMS method. Five stars means much above average. The overall score is not a simple average of the three parts.
Health inspections draw on standard surveys and complaint investigations. In Arizona, state surveyors work under CMS with ADHS involvement on the licensing side.
Quality measures include falls with major injury, pressure ulcers, infections, and related clinical outcomes. Those measures are not gossip. They are federal fields.
Staffing numbers families can actually read
CMS collects staffing through the Payroll-Based Journal system. Facilities submit quarterly payroll data for RN, LPN, and CNA hours.
The national average for total nursing staff hours was 3.68 per resident day in Q1 2024 PBJ data. Compare a facility against that benchmark with current figures.
Start with RN hours per resident day. Registered nurses handle assessments and clinical decisions. A thin RN line shows up in missed changes of condition.
Look at weekend staffing next. Many homes drop coverage on weekends. A weekday number that looks fine can hide a weekend gap.
Turnover rates are public too. High turnover means new staff who do not know your person. That is a care fact, not a slogan.
Arizona nursing homes as a group average fewer registered nurse hours per resident day than. The national median in the investigation’s staffing read. Individual buildings still need their own lookup.
Penalties, citations, and what the letters mean
When surveyors find a problem, they issue a deficiency citation. Each citation carries scope and severity.
Scope runs isolated, pattern, or widespread. Severity runs from potential for minimal harm up to immediate jeopardy.
Immediate jeopardy means a situation that has caused or is likely to cause serious injury, harm, impairment, or death. That label triggers faster federal enforcement pressure.
Citations at level G and above indicate actual harm. Families can read the narrative, not only the letter code.
Penalties include civil money fines and payment denials for new admissions. In severe cases CMS can terminate Medicare and Medicaid certification. AZ CareCheck at azcarecheck.azdhs.gov is the state licensing lookup path that pairs with the federal profile.
Who else can receive a report
Adult Protective Services takes reports on vulnerable adults through the Arizona Department of Economic Security. CMS-style survey complaints and APS reports are different tracks.
You can document first and report on more than one track if the facts warrant it. Civil questions and licensing questions are not the same door.
Arizona Revised Statutes 46-451 defines a vulnerable adult for related protective statutes. Families do not need to recite the statute to ask for safe care and complete records.
If your person cannot speak for themselves, bring the power of attorney papers. Ask the facility to note who is authorized to receive updates.
Do not sign broad releases from an insurer or facility risk office before you understand the scope. Read every line.
A night-shift fall checklist
- See your person and photograph injuries.
- Write the first staff explanation with times.
- Request the incident report and full chart.
- Save ER and ambulance records.
- Pull CMS Care Compare and screenshot ratings.
- Read staffing and inspection pages, not only stars.
- Check AZ CareCheck for state licensing history.
- Ask that camera footage be preserved if cameras exist.
- Start one dated folder the same night.
- Decide on APS or survey complaints after the person is safe.
Keep the records families request after a nursing-home fall next to the police report and the first photos.
Use that investigation as a map to public files, not as a verdict about one building. Standard surveys happen about every twelve to fifteen months. Complaint investigations can move faster when immediate jeopardy is alleged.
Your job tonight is narrower. Get care. Get copies. Get dates. The folder you start on the night shift is still the folder that makes sense at breakfast.
